                                                      ABSTRACT

Health and safety is a term that generally covers the legal requirement that fall under the health and safety at work Act 1974. Health and safety is generally used to describe Occupational Health and Safety, and relates to the prevention of of accidents and ill health to employees and those who may be affected by their work.

In organizations today, human resources have become the major asset for productivity in its maximum since machines do not have the capacity to operate it selves. In Kano state, many health workers lacked the right information to work safely which has exposed many employees to potential risk and workplace hazard that may have a partial or permanent disability and sometime may result to fatality at work site, this could be as a result of ignorance to the correct practice or people still trying to gain earns meet for survival due to the low job opportunity in the state.

The title of this research work is “The effect of Health and Safety on Employee’s Productivity in Kano state Hospital Facilities” The study will highlight the standards operational procedure at workplace to enlighten hospital workers the duty of care of the employers to them while they carry out an assigned job, the paper will also cover the identification of health risk and safety risk and the required preventive controls to reduce risk to as low as reasonably practicable in the workplace, safe work environment and practices are major factors to be considered before accepting job offers. The research was carried out at Kano state, Nigeria. The research reveals the selected hospitals has a working occupational health and safety practices but has an average management commitment to the safe work practices of employees. The data was collected via questionnaire using a four Likert scale procedures for analysis and descriptive survey method. This study is majorly concerned with the impact of occupational health and safety on the productivity of employees.

INTRODUCTION

In recent years, the subject of occupational health in Nigeria has gained more daily audiences with both employees and their employers.
Workplace wellness isn’t any longer a choice. This aspect is essential for ensuring maximum productivity while preserving lives and property.

Today’s organizations face challenges that impact profitability, consumer base, productivity, and employee wellness. Occupational wellness is vital in this environment since employees are the most prominent part of the organization.

Occupational health and safety cover the physical, mental, and social well-being of people at work, including issues such as stress prevention, human resource development, and rehabilitation. The state has a regulatory framework that comprises laws and regulations enacted by the State Government regarding occupational health and safety.

In this paper, we discuss occupational health and safety in kano state. Additionally, we will provide some general guidelines on OHS that apply anywhere in the world.

 

This part of the study entails the background of the study which captured the effect of health and safety standards on employee’s productivity in Kano state and how it can boost higher performance. The study also covers statement of the problem, purpose of the study, significance of the study, organization of the study and limitation of the study.

 

 

BACKGROUND TO THE STUDY

Occupational health and safety is a process of ensuring that people stay safe and healthy in the workplace to increase workers capacity to perform. The workplace has become an integral part to the viability of business for employers, labour union, the society as well as the government. The quality of the workplace environment has an impact on the level of employees motivation and performance. How well employees engage with the organization, especially the immediate environment influences to a great extent their error rate, level of innovation and collaboration with other employees as well as absenteeism and ultimately affect the number of years they stay on the job. Health and safety standards instituted by organizations is aimed at the promotion and maintenance of the highest degree of physical, mental, social well being of workers in all sectors that have an impact on the health of employees.

To have a greater knowledge on employee’s health and safety, it would be required to distinguish between the two parties:

Safety is mainly concerned with the prevention aspect of diseases or accident, the aim being to ensure that any accident or diseases does not occur due to ignorance or inability of the employers or employees to forestall such occurrences.

While employees health on the other hand, ensures that the outbreak of any diseases occurrence is curtailed and adequately handled.

According to some big organizations like WHO and International Labour Organization, occupational health as an area of work in public health to promote and maintain the highest ‌physical, mental, and social well-being of workers in all occupations. Occupational health and safety (OHS) management systems identify, analyze, control, or eliminate the risks to people’s health and safety that arise from their work activities” (WHO, 2022)

And International Labour Organization (ILO) defines occupational health as; “the promotion and maintenance of the highest ‌physical, mental, and social well-being of workers in all occupations. By preventing departures from health, controlling risks, and the adaptation of work to people, and people to their jobs”. (ILO- International Labour Organization, 2022)

 

This study focuses on “employee’s health workers’ productivity” productivity can simply be defined as the relationship between the output generated and by a production or service system and the input provided to create this output. Industrial accident and diseases lead to loss of working days, which could mean decrease in productivity.

Health and safety of employees is therefore an important issue which if studied carefully and thoroughly, could be of great benefit to the organization as well as the nation at large. Thus, the improvement of health and safety measures could yield the best result in terms of productivity and general development of the nation.

 

PROBLEM STATEMENT

In recent years some workers have temporarily or permanently been disabled by work related accidents as a result of inadequate occupational health and safety standard practice and knowledge on handling of machines and equipment, negligence to safe operational procedures and poor management commitment to providing a friendly work environment. Every business has the legal responsibility to ensure the health and safety of employees and other people affected by the business activities such as Patients, suppliers, visitors and contractors. Poor health and safety practices lead to illness, accidents, fatality and significant cost to the business. Effective health and safety practices pay for themselves and improve the reputation of the organisations with customers, regulators and the employees. The human resource is one of the versatile resources a hospital depends on for efficient care delivery. Therefore, an effective and efficient use of the human resource will translate into the overall effectiveness and efficiency of the Hospital. Although, many Hospitals accept the human resource as an important resource but they fails to realize that as part of their human resource management practices, there is the need for management to ensure that personnel in the hospital work in safe and healthy environment will promote their optimum utilization. It should be emphasized that accidents are costly both to the affected worker, Hospital, Government and public in general. Therefore, every effort should be made by Hospital management, government and employees in order to avoid occupational accidents from occurring.

 

 

The Occupational Health And Safety Act In Nigeria

As an affirmation that safe working conditions are essential to attaining social justice and economic growth, the Federal Government of Nigeria ratified International Labour Organization’s ILO Convention No. 155 on Occupational Safety, Health, and Working Environment in 1994. According to Article 17.3.c of the 1999 Nigerian Constitution (as amended), the government ensures and protects the health, safety, and welfare of all employees; the National Industrial Court of Nigeria Act and the OSH Policy make this law.

Major legislation that has been enacted to provide for the safety and health of the workforce include:

· Employees’ Compensation Act, 2010: The employees’ Compensation Act, Cap. It makes comprehensive provisions for the payment of compensation to workers who are injured at work or suffer occupational diseases.


RELEVANCE TO PUBLIC HEALTH

The term “public health” refers to the health of the population as a whole, especially as the subject of government regulation and support. The term is also used to refer to the branch of medical science dealing with public health. Occupational safety and health, meanwhile, refers to the safety and health of workers, especially as the subject of preventive and protective measures put in place by employers, and based on government regulation. These basic definitions point at the evident causal link between the two fields: occupational health is an important determinant of public health, for work can be and unfortunately often is a cause of diseases. In other words, work, exposure to occupational risks, and working conditions are essential factors for understanding population health. Citizens and workers are the same people: if they work in bad conditions that affect their health, it will show in public health statistics through increased disease rates.

General objectives:

The main objective is to assess the knowledge, perception and practice of health and safety standard among health workers in Kano state Hospitals


Specific objectives:

To assess the knowledge of health and safety standards among health workers in Murtala Muhammad Hospitals and Nasarawa Hospital as a case study

To assess the perception of health and safety standards among health workers in Murtala Muhammad Hospitals and Nasarawa Hospital as a case study

To assess the practice of health and safety standards among health workers in Murtala Muhammad Hospitals and Nasarawa Hospital as a case study

To identify the factors determining the practice of health and safety standards among health workers in Murtala Muhammad Hospitals and Nasarawa Hospital as a case study.


HYPOTHESIS

1.    H1: Employees health measures are not independent of organisational   productivity?

2.    H2: Safety measures cannot be used independently as a criterion for reduction of accident in work environment?.

4.    H4: A safety environment is not independent of employee’s motivation?

 

SAFETY AND PRODUCTIVITY

Over the past crazy year, organisations have learned many important lessons—some, the hard way.

From these many lessons that we are still experiencing, managers and supervisors have seen how ongoing, well-planned safety impacts productivity and quality of work. However, it is a major misconception that workplace safety and productivity are mutually exclusive, and in order for one area the benefit, the other must be negatively impacted. The reality is that safety and productivity actually complement each other.

When safety improves, so does productivity. In fact, each priority feeds each other in ways that have become clearer with the recent increase in remote and telework. According to a recent study in Occupational Health Science, “employees’ safety and health are of primary importance, as both are key elements in achieving an organization’s desired productivity and efficiency.” Furthermore, according to an article from Automation World, “manufacturers need to view safety and productivity as goals to achieve together, rather than ‘safety first’ after productivity.”

When a workplace or the work itself is unsafe, it means that employees may not be well-managed. That could result in less motivated and mindful employees who are unsatisfied and unhappy with their job. Poor safety management can not only put the team members’ safety on the line, but also operations and productivity. Additionally, heavy fines can result if litigation takes place, crippling—if not completely shutting down—an organization.

When safety is a priority and there’s a culture of well-being within the workplace (even online), team members can focus on the quality of their work instead of the worry and stress that comes with potentially hazardous work environments. A recent study from Science Direct found that physical and psychosocial conditions at work directly impact worker safety, health and well-being and “influence enterprise outcomes such as turnover, absence, productivity and healthcare costs.”

The effect of health and safety on employee productivity is significant. When employers prioritize health and safety in the workplace, employees are able to work more efficiently and effectively. Here are a few reasons why:

1. Reduced Absenteeism: Employees who feel safe and healthy in their workplace are less likely to call in sick or take time off due to illness or injury. This means more employees are present and available to work.

2. Improved Morale: When employees feel that their employer values their safety and well-being, they are more likely to feel satisfied and motivated in their work. This positive attitude can translate into a better work ethic and a higher level of productivity.

3. Better Performance: When employees are not worried about their safety or health, they can focus on their work more effectively. They are less likely to make mistakes, and more likely to produce high-quality work at a faster pace.

4. Greater Retention: Employees who feel that their employer prioritizes their health and safety are more likely to stay with the company for a longer period of time. This reduces turnover rates and helps ensure that the most experienced and qualified employees are retained.

Overall, prioritizing health and safety in the workplace can have a positive impact on employee productivity and can help businesses operate more efficiently.


METHODOLOGY

STUDY AREA

This research was conducted to determine the effect of occupational safety and health

on employee productivity at Murtala Muhammad Hospital and Nasarawa Hospital. 

 

STUDY POPULATION

The population in this study comprises of employees of the Murtala Muhammad Specialist Hospital and Abdullahi Wasai Teaching Hospital amounting to 200 staffs

Inclusion Criteria: Including both Clinical and non Clinical staffs.

Exclusion Criteria: No exclusion


SAMPLE SIZE DETERMINATION

While the sample selection in this study was carried out using the Non probability Sampling technique, which is a sampling technique that does not provide equal opportunity / opportunity for each element or member of the population to be selected as a sample.


SAMPLE TECHNIQUE

The sampling technique used in this study is the saturation sampling technique. Saturated sampling is a sampling technique when all members of the population are used as samples.


DATA COLLECTION METHOD

The number of samples used in this study was 200 people. The data source in this study is primary data obtained by conducting research directly on the research object at Murtala Muhammad Hospital and Nasarawa Hospital which is done through observation, interviews, and distribution of questionnaires to respondents or employees of the Murtala Muhammad Hospital and Nasarawa Hospital. 

 

Observation, the researcher makes direct observations, especially on the work safety and health of employees at Murtala Muhammad Hospital and Nasarawa Hospital. 

Interview is a method of data collection by conducting direct interviews with Medical Officers, Nurses and Patients to obtain the required information. And questionnaires are data collection techniques through questionnaires submitted to respondents. To obtain numerical data from the questionnaire the questionnaire answers were entered into a Likert scale of scores 1-5.

The data analysis method is done by multiple linear regression analysis formulated as Y = a + b1X1 + b2X2 where Y is work productivity, X1 is work safety and X2 is work health. And to test the hypothesis, several tests are carried out, namely: a) Partial Significant Test (t test), this test is carried out to find out how far the influence of one independent variable partially (individually) explains the dependent variable. The criteria for decision making are Ho is accepted if tcount <ttable at (α) = 5% and Ha is accepted if tcount> ttable at (α) = 5%. 2) Simultaneous Significant Test (F test), this test is carried out to see all the independent variables entered in the model have a simultaneous effect on the dependent variable. Where the decision making criteria is that Ho is accepted if tcount <ttable at (α) = 5% and Ha is

accepted if tcount> tcount at (α) = 5%. 3) Coefficient of Determination (R2), the coefficient of determination (R2) basically measures how the model's ability to explain the effect of independent variables on the dependent variable where, 0<=R2>= 1. If R2 is greater (close to one), then it can be said that the influence of variables free is large towards the dependent variable. Conversely, if R2 gets smaller (near zero), it can be said that the effect of the independent variable is small on the dependent variable.

 

RESULT

BASELINE SOCIO-DEMOGRAPHIC DATA

Based on the results of regression analysis with the help of SPSS software Y results

obtained are 2.255 + 0.314X1 + 0.811X2. This regression model can be explained that the value of βo which means a constant number of 2.225 shows that without the factors of occupational safety and occupational health, the score for employee productivity is 2.225. The β1 value of 0.314 shows the meaning that if there is a change in the dimensions of work safety (X1), the employee's work productivity (Y) will experience a positive increase of 0.314. A β2 value of 0.811 shows the meaning that if there is a change in the dimensions of occupational health (X2) then employee work productivity (Y) will experience a positive increase of 0.811. And the coefficient of determination of 0.614 or 61.4%. This shows that the variable of work productivity (Y) is influenced by the variables of occupational safety (X1) and occupational health (X2) of 0.614 or 61.4%. While the remaining 0.396 or 39.6% is influenced by other variables outside this research model.

For the partial test, it was obtained that the tcount for the work safety variable was 2.333 while the ttable was 1.68195 then the tcount was 2.333> t table 1.68195 with a significant level (0.025 <0.05) meaning that the work safety variable (X1) had a positive effect and significant effect on employee productivity (Y). And the value of tcount for occupational health variables is 8.063 while the value of ttable is 1.68195 then the tcount is 8.063> 1.68195 with a significant level (0,000 <0.05) meaning the occupational health variable (X2) has a positive and significant effect on work productivity (Y) employee. As well as the simultaneous test results obtained that indicated Fcount of 33.444 while Ftable with a significance level of 0.05 amounted

to 3.22 then Fcount> Ftable (33.444> 3.22) with a significant level of 0.000 <0.05 so it can be concluded that the work safety variable and work health simultaneously (together) have a positive and significant effect on employee work productivity in the Federal Medical Center, Federal Capital Territory Abuja Nigeria.

 

DISCUSSION

Based on the results of the distribution of questionnaires to respondents by submitting

several statements, the results obtained on the variable work safety: respondents' responses regarding "Work Safety" in Murtala Muhammad Hospital and Nasarawa Hospital is as follows:

Respondent's response to the statement "The hospital provides work protective equipment such as Nose mask, Hand sanitizers, and gloves, in accordance with work safety standards" in the category of disagreeing as much as 1 respondent with a percentage of 2.2%, which stated agree as much as 26 respondents with a percentage of 67.8%, who stated strongly agree as many as 18 respondents with a percentage of 40%. Thus the highest respondent is agreed.

The second statement "All work equipment is in good and proper condition" in the category of No is 1 respondent with a percentage of 2.2%, who states yes as many as 22 respondents with a percentage of 48.9%, and who states Yes as many as 22

respondents with a percentage of 48 9%. Thus the highest respondent is yes and strongly agreed. The third statement "All parts of hazardous equipment have been given a warning sign" in the category of agree as many as 22 respondents with a percentage of 48.9% and who strongly agree as many as 23 respondents with a percentage of 51.1%. Thus the highest respondent is very agreeing. The fourth statement "Every employee who works in a safe working environment" in the category of disagreement is 2 respondents with a percentage of 4.4%, who states agree as many as 22 respondents with a percentage of 48.9%, and who states strongly agree as many as 21 respondents with the percentage of 46.7%. Thus the highest respondent is agreed. The fifth statement "The Hospital conducts more intensive supervision of the implementation of your work" in the category of No to 10 respondents with a percentage of 22.2%, and which states yes as many as 35 respondents with a percentage of 77.8%. Thus the highest respondent is Yes. The sixth statement "The Hospital provides a guarantee of safety insurance at work" in the category of agree as many as 10 respondents with a percentage of 22.2%, and who stated strongly agree as many as 35 respondents with a percentage of 77.8%. Thus the highest respondent is very agreeing.

Work health variables obtained responses to the statement "The environment in which you work has adequate air ventilation and lighting". In the category of disagreeing as much as 1 respondent with a percentage of 2.2%, who stated agree as much as 27 respondents with a percentage of 60%, and who stated strongly agree as much as 17 respondents with a percentage of 37.8%. Thus the highest respondent is agreed. The second statement "The Hospital provides health insurance to every employee" in the category of agreed as many as 28 respondents with a percentage of 62.2%, and which states strongly agree 17 respondents with a percentage of 37.8%. Thus the highest responder is agreed. The third statement "The environment where you work in a clean condition so that it has a good influence on the continuity of employee work" in the category of disagreeing as much as 1 respondent with a percentage of 2.2%, which states less agree as much as 7 respondents with a percentage of 15.6%, who agree as many as 28 respondents with a percentage of 62.2%, and those who stated strongly agree as many as 9 respondents with a percentage of 20%. Thus the highest respondent is agreed. And work productivity variable obtained response statement "I am able to carry out all the work in accordance with the capabilities that I have" in the category of not agree as much as 1 respondent with a percentage of 2.2%, which states agree as many as 29 respondents with a percentage of 64.4%, and which states strongly agree as many as 15 respondents with a percentage of 33.3%. Thus the highest respondent is agreed. The second statement "The work that I endeavored to increase work productivity" in the category of agree as much as 32 respondents with a percentage of 71.1%, and who stated strongly agree as many as 13 respondents with a percentage of 28.9%. Thus the highest respondent is agreed. The third statement "I was able to complete the job according to the schedule set" in the category of disagreeing as much as 1 respondent with a percentage of 2.2%, which stated less agree as much as 2 respondents with a percentage of 4.4%, who agreed as much as 29 respondents with a percentage of 64, 4%, and those who strongly agreed were 13 respondents with a

percentage of 28.9%. Thus the highest respondent is agreed. The fourth statement "In completing work, I am very concerned about the quality of the care i provide" in the category of agree as many as 32 respondents with a percentage of 71.1%, and those who stated strongly agree as many as 13 respondents with a percentage of 28.9%. Thus the highest respondent is agreed.

 

Solution

How To Stay Safe On The Job As A Health Worker

While many of us do not work in “risky” environments, health workers are the first responders to any outbreaks and are exposed to hazards that can expose them to outbreak pathogens (in this case COVID-19). It’s important to pay attention to what we can do to protect ourselves from unsafe conditions.

Health workers can stay safe on the job by:

· Participating in occupational health and safety training provided by the employer, besides following established occupational safety and health procedures.

· Assessing and treating patients according to the provided protocols.

· Report suspected and confirmed viral infections and diseases under public health reporting procedures.

· Personal protective equipment should be worn, taken off, and disposed of properly.

· Self-monitor for signs of illness and self-isolate or report the illness to managers if it occurs.

· Where they have reasonable justification to believe that life or health is at risk, they should immediately contact their immediate supervisor.  

Health facilities, employers, and managers must ensure all health workers have the right to safety by:

· Assuming overall responsibility to prevent occupational injury and disease.

· Workplace safety and health information and training, including infection prevention and control, as well as instruction on how to use, wear, remove, and dispose of personal protective equipment (PPE).

· Supply sufficient supplies (masks, gloves, goggles, gowns, hand sanitizer, soap and water, cleaning supplies) to healthcare professionals caring for suspected Covid-19 patients or confirmed patients.

· Provide workers with technical updates on COVID-19, provide tools for assessing, triaging, testing, and treating patients, as well as communicate infection prevention and control information with patients and the public.

· Use personal safety and security measures as needed.

· Provide an environment that is blame-free for employees to report incidents involving blood or bodily fluid contacts, and take the ‌steps, including aid for victims.

· Encourage employees to evaluate themselves, report symptoms, and stay at home when they’re sick.

· Consult with health workers on the safety and health aspects of their jobs, and notify the labor inspectorate of any incidents of occupational diseases.

 

Indicators Of Occupational Health

Occupational health is a preventive medicine approach, which aims to optimize workplace design or working conditions to allow workers to be healthy. The goal of occupational health programs is not only to reduce disease and injury rates but also to enhance productivity and well-being.
Many examples can help you determine how employers and employees can take what kind of action on an individual level to ensure that they are happy at work. They include:

· Policy development: A successful safety and health program requires a comprehensive policy that not only meets national and international standards but also contributes to the overall performance of your health system.

· Government—responsibility and accountability: The government must assign responsible, accountable, and authoritative parties to develop, implement, and execute the safety and health policy. This ranges from ensuring that safety and health is a line management responsibility that is known at all levels to establishing effective arrangements to identify, measure (if possible), control work-related hazards and risks, and promote health.

· Identification and controlling of workplace hazards: This requires careful planning and coordination to be successful. This involves an inspection of the facility using a checklist to identify recognizable and unrecognizable OH hazards and ‌helps you manage workplace risks.

· Emergency prevention, preparedness, and response: Preparing and training employees, employers, managers, and supervisors before an emergency incident plays a vital role in ensuring that employers and workers have the ‌equipment, know where to go, and know how to keep themselves safe when an emergency occurs in the workplace. 

· On-site evaluation and surveillance: It is imperative to monitor your safety and health performance regularly all too often, good start and enthusiasm wane, and standards deteriorate. It is the government’s responsibility to establish procedures for monitoring, measuring, and recording safety and health performance.

 

CONCLUSION 

Based on the results of the analysis and discussion that has been carried out, it can be

concluded that from the results of the multiple linear regression analysis test that the level of work safety has a positive effect on employee productivity, the results show that the regression coefficient for work safety (X1) is 0.314. Mean work productivity (Y) in Kano state will increase by 0.314 if the effort to increase work safety goes up by one unit. And from the results of multiple linear regression analysis test that the level of occupational health there is a positive influence on employee productivity, the results show that the regression coefficient for work health (X2) of 0.811. Mean work productivity (Y) Kano state Hospitals will increase by 0.811 if the efforts to improve work health go up by one unit.
